アカウントホルダーが、異議申し立てをするためのフォームです。

異議申し立てを行なってください。
法令に基づき、速やかな対応が行われます。

アカウントホルダーが著作権や配信権を有していることを、
証拠書類を添付の上、明確に証明する必要があります。
・このフォームでは申立てに対して、著作権や配信権を有しているかどうかを
判断します。証拠となる資料を一式提出いただく必要があります。
一件ずつ判断を行うため、時間を要することがあります。
・電子署名欄に氏名を入力することで、電子署名として使用します。
